This plan will allow us to address preexisting health concerns, but most importantly, it will allow us to identify potential risks and make the lifestyle changes necessary to keep you happy and healthy!

Upon processing your request, we will invite you to register for our patient portal and to fill out the new patient paperwork
Your Virtual Meet & Greet will be scheduled via email.
If, at the conclusion of your M&G, you decide that we would be a good fit for each other, we will schedule your initial Establish Care appointment with your new Primary Care Provider.

We have these steps in place for several reasons:

We are a small family business and all of our providers are APRNs. As such, there are certain cases where we would not be the appropriate solution for your healthcare needs. If you have an extremely complex medical condition/situation, we would not be the right solution for you.
We take great pride in being available to our patients, so we are very cautious as to how large we allow our panel to grow. We ensure an efficient onboarding process to allow for steady consistent growth while maintaining our community-leading patient-to-provider ratio.
We do conduct a majority of our visits via telehealth and all of our patient forms are completed electronically. If you are unable or unwilling to attend ZOOM appointments or fill out paperwork electronically, we would not be the right solution for you.

We do accept keikis (ages 3 & up) but require a review of the patient's full medical and immunization records. Because we are not pediatricians, we take great care to ensure we are an appropriate caregiver for your Keiki. Please be prepared to provide these records before requesting a Meet & Greet.

If you do not have the ability to complete online registration, please give our office a call to be placed on the wait list for in-person registrations. Please, have your insurance information ready.

The online registration process works best on a laptop or desktop computer using a Chrome browser. Registration on a mobile device is not currently supported.

We are unable to participate in any HMO plans or any HMAA plans at this time.
We do not process claims or paperwork for Workman’s Comp, Auto Insurance, or Disability.
